Community Midwives

Midwives are allocated to the Practice by Derby Hospitals NHS Foundation Trust. They arrange booking to the hospital of choice for delivery, carry out the majority of antenatal care at the surgeries and visit patients at home after their baby has been born. Patients are invited to bring their baby to the surgery for an appointment with a GP when they are 10 days old for a baby check.

Community Nurses

Our District Nurse and therapy team, who are employed by Derbyshire Community Healthcare Services (DCHS). They are able to visit the housebound and people with palliative care needs to offer a range of nursing care when this is needed. They are based at Ripley Community Hospital but can be contacted through the surgery.

Care Coordinator Role:

Care Coordinators are part of the Integrated Community Team employed by Derbyshire Community Health Services NHS Foundation Trust and are based in GP practice. We are part of the Care Support Team and work closely with the GP’s at your practice and the Community Matron who helps patients with long term conditions.

Having a multi-agency approach the Community Support Team consists of a Care Coordinator, GP, Community Matron, Community Nurse, Community Therapist, the Community Mental Health Team and Adult Care. We hold regular multi-disciplinary team meetings to ensure a smooth pathway of care for our Patients.

This is a non-clinical role and we liaise with Patients, Carers and their Families to support people to stay safely at home wherever possible by having a person centred approach and providing information / signposting and referring to relevant health, social care and voluntary services if appropriate.

I can be contacted through your GP Practice.
