Repeat prescriptions need to be requested in writing, and the easiest way to do this is by using the online prescription request form below. Alternatively you can use the tear-off slip attached to your previous prescription. Tick each item you need and hand this in at the surgery or to your pharmacist, or post it to us. If this is lost, simply write down the items you require.

Please allow us two working days before coming to collect a routine repeat prescription from us. We ask for the request in writing to minimise the possibility of a mistake occurring.

Some local chemists offer a collection service for prescriptions when they collect from the surgery and make up your prescription ready to collect. Please ask at reception or your local chemist if you think this may help. If you let us have a stamped, addressed envelope we will post your prescription on to you but please allow extra time for this.
